Privacy: Atlas Chiropractic collects and stores your contact and health information to provide your care, manage your appointments, and meet our legal obligations under the Privacy Act 1988 (Cth) and the Health Records Act 2001 (Vic).
We use a small number of service providers to do this: Cliniko (practice management and clinical records), Stripe (payments), Mobile Message (appointment text messages) and Render (hosting for our website and booking system). When you text us about your appointments, an AI assistant (Claude, provided by Anthropic) may read your message and write and send the reply automatically. At your request it can book, move or cancel your appointment. It never gives clinical advice: anything about your health, fees or a concern is passed to Shaun, who replies personally. Some of these providers store or process information outside Australia, including in the United States. We only give each provider what it needs to do its job.
We don't otherwise share your information except where needed for your care (e.g. a referral, with your consent), a Medicare/health fund claim, or where required by law. You can ask to access or correct your information at any time, or raise a concern with us directly, the Health Complaints Commissioner (Vic), or the OAIC. Contact: info@atlaschirohc.com.au
Consent to treatment: chiropractic care may include spinal and joint manipulation/adjustment, mobilisation, soft tissue therapy, dry needling, and exercise or lifestyle advice. Like any healthcare treatment it carries some risk, common, temporary effects include soreness, stiffness or fatigue after treatment; rare risks include muscle or ligament strain and, very rarely, rib fracture or injury to a neck artery associated with cervical manipulation, which can potentially lead to stroke. No treatment outcome can be guaranteed. Your practitioner will discuss any risks specific to you and is happy to answer questions before, during, or after treatment. You are free to decline any part of your treatment or withdraw your consent at any time, including partway through a session, without affecting your ongoing care.
Fees & cancellations: your session fee is charged to the card held on file once your appointment has taken place. A cancellation with less than 12 hours' notice, or a missed appointment, may incur a fee of up to 50% of the session cost, charged to the card held on file.
Your rights: you may ask questions about your care at any time, request a chaperone during examination or treatment, decline any part of a proposed treatment, or withdraw your consent altogether, without needing to give a reason. If you have a concern about your care or this consent form, you can raise it directly with Atlas Chiropractic at info@atlaschirohc.com.au, or with the Health Complaints Commissioner (Vic) or AHPRA.
Photography & filming (optional): from time to time, Atlas Chiropractic may want to photograph or film part of a treatment session for educational or marketing purposes (for example, on social media or the practice website). This is entirely optional and separate from your consent to treatment below. Declining will not affect your care in any way, and you can change your answer at any time by contacting the clinic.
